What Is an Intensive Outpatient Program?

An intensive outpatient program (IOP) is a structured level of addiction treatment that provides several hours of therapy per week without requiring you to live at a treatment facility. IOP falls between a partial hospitalization program (PHP) and standard outpatient care on the continuum of treatment.

IOP is recognized by insurance providers and the American Society of Addiction Medicine (ASAM) as an appropriate level of care for individuals who need consistent clinical support but have enough stability to manage daily responsibilities outside of treatment. Research consistently shows that intensive outpatient treatment is as effective as inpatient rehab for many patients.

At Kolmac, IOP combines group therapy, individual counseling, psychiatric services, medication-assisted treatment (MAT), and family participation into a single, integrated treatment plan.

What to Expect in Kolmac’s IOP

Kolmac’s intensive outpatient program is built around group therapy as the core treatment modality, supplemented by individual clinical and medical care.

Three Phases of Kolmac IOP Treatment

Kolmac’s intensive outpatient program follows a structured, three-phase approach that ensures a smooth transition from stabilization through long-term recovery support.

Phase 1: Detoxification. For patients who need medically supervised withdrawal management, Kolmac offers virtual, home-based detox. Our medical team monitors you throughout the process, adjusting medications as needed to keep you safe and comfortable before you begin IOP.

Phase 2: Intensive Outpatient Program (IOP). The core treatment phase combines group therapy, individual counseling, psychiatric services, and skill-building. This is where the daily work of recovery happens while you continue living at home and maintaining your responsibilities.

Phase 3: Outpatient and Continuing Care. After stabilization, patients step down to a less intensive outpatient schedule that includes group sessions, individual therapy, family participation, and ongoing medical support. Patients can participate in continuing care in-person, fully virtual, or in a hybrid format.

Who Is IOP Right For?

Kolmac’s intensive outpatient program is appropriate for individuals who:

  • Have a stable and supportive home environment
  • Need structured treatment but can manage daily responsibilities alongside care
  • Are stepping down from a partial hospitalization program (PHP) or inpatient rehab
  • Prefer to continue working, attending school, or caregiving during treatment
  • Are committed to long-term recovery and ready to engage in group-based treatment

If you are unsure whether IOP is the right level of care, Kolmac’s clinical team will determine the appropriate placement through an initial evaluation. Some patients begin in PHP and transition to IOP as they stabilize, while others enter IOP directly.

Family Participation

Addiction affects the whole family, and recovery is stronger when families are involved. Kolmac offers weekly family group sessions (all virtual, three options per week) where loved ones can learn about addiction, understand the recovery process, and build healthier communication patterns.

We also encourage participation in community support groups such as Al-Anon, Nar-Anon, Alcoholics Anonymous, Narcotics Anonymous, and SMART Recovery Family and Friends.

IOP vs. PHP: Choosing the Right Level of Care

The main difference between an intensive outpatient program and a partial hospitalization program is the number of hours per week.

IOP involves 3 hours per day, 4 days per week (12 hours/week). It is appropriate for patients who have enough stability to manage daily responsibilities alongside treatment, are stepping down from PHP, or are entering treatment for the first time with a supportive home environment.

PHP involves 6 hours per day, 5 days per week (30 hours/week). It is appropriate for patients who need a higher level of structure, are earlier in their recovery, or have co-occurring mental health conditions that require close monitoring.

At Kolmac, many patients begin in PHP and transition to IOP as they stabilize. Our clinical team determines the appropriate level of care through an initial evaluation.

Virtual IOP and Telehealth Options

In addition to in-person IOP at all Kolmac locations, we offer virtual IOP through secure telehealth. Virtual IOP is available across Pennsylvania, Maryland, Virginia, and Washington, D.C., and provides the same evidence-based treatment as our in-person program.

Virtual IOP is ideal for patients who live farther from a Kolmac location, have transportation barriers, or prefer the convenience of attending sessions from home.
